For each category you may nominate as many people as you feel are appropriate -- please only nominate each person once.

Note: under no circumstances are doulas or affiliate members of DASC to solicit nominations -- those who do will be disqualified.

You do not need to nominate someone in each category. Forms must be submitted by August 15th (no late entries will be accepted). Please include the address or home address, if known, to ensure proper delivery and attendance at the 2012 event. Invitations mailed to hospitals have not been received by the nominees in past years!

After filling in a nomination form, you will be directed back to this page to nominate in another category.

When considering who to nominate, consider how this person inspired you, supported you and/or your client, modified their traditional protocols to give your client the opportunity to birth her way, etc.   You are NOT necessarily saying this person is the BEST in the category for the entire southern California area ... just that they deserve recognition for their care and support.
